Group Adult Foster Care, also known as GAFC, is a supportive care program for eligible adults who need help with daily living activities and want to remain in a home or community-based setting.

Through RMG Renaissance Medical Group, GAFC provides care coordination, support, and guidance for members and caregivers. The program is designed to promote safety, dignity, independence, and stability while helping families feel less alone in the caregiving process.

GAFC is not just about assistance. It is about helping people live with respect, comfort, and support in the place they call home.

RMG guides families through the process:

  1. Program Inquiry
    The member, caregiver, family, or referral partner contacts RMG.
  2. Eligibility Review
    RMG reviews the person’s needs and explains possible program requirements.
  3. Assessment and Care Planning
    A care plan is developed based on the member’s daily needs, safety, and support goals.
  4. Caregiver Support
    The caregiver receives guidance and support to help care for the member.
  5. Ongoing Monitoring
    RMG provides continued communication, care coordination, and follow-up.
